Services In-Home Family Support provides training and support to a person and his/her family,
including extended family, in the family home and in the community.
Services are designed to increase the family's ability to care for and
support the person in the family home. Personal Support Personal Support helps a person increase independence, productivity and
inclusion in the community. Personal support services provide more flexible
and less formal, or less intensive support than supported living services. Respite Care Respite Care provides short-term care to a person when the family member(s)
or primary caregiver cannot be there or needs a rest from his/her responsibilities.
Respite care may be provided in the persons home or in a different residence that
has been approved by the county. Respite care may include day and overnight
services. Semi-Independent Living Services (SILS) SILS are provided to adults with mental retardation or related conditions who
require less than 24 hours of supervision and are able to maintain their own homes
in the community with support and assistance. SILS includes training and assistance
in managing money, maintaining personal appearance and hygiene, preparing meals,
shopping, and other areas. Supported Living Services (SLS) Supported Living Services are residential based habilitation services provided to children or adults
who require up to and including 24 hour supervision, training or assistance
on an on-going and daily basis. Supported Living Services may be provided in the person's own home,
apartment or in one of the Adult Foster Homes owned by Great River Homes, Inc.
